Appointments cannot be performed earlier than scheduled. Late arrivals may need to be rescheduled.

Arrive on time.  

Payment

Payment is due at the time of service for personal DOT physicals.  We accept cash, credit, debit, and Apple Pay.  DOT consortiums / TPAs are not billed for physicals.

Bring a valid state or federal ID. Expired IDs, copies, or phone images are not accepted.

Medical Info

Prepare a list of current medications (name, dosage, frequency).  Have the dates of any surgeries or notes from any recent treatment from your doctor.  If you use a CPAP, print and bring in the last year's compliance report. You will receive a vision and hearing test during your exam.  It is a simple check-up, but if you wear glasses or hearing aids while driving, you need to bring those with you for this portion of the exam.  

Urine Specimen  

DMV exams do not require a drug test.  Unless you are aware of a drug test that is needed at the time of your exam, you will not be required to take one.  The doctor will need a urine sample for a basic urinalysis.  Please drink a water before arriving to ensure you can provide a specimen in a timely manner.

1.  What do I do after the exam?    

You will receive your completed exam paperwork and signed certificate at the end of your visit. It is your responsibility to submit these documents to the DMV.

2.  Can you send results to my company or another party?    

Your exam and medical information are confidential.  We can only release results with your written consent.

3.  Can I get additional copies of my DMV Certificate?   

Yes. You will receive your original paperwork along with one additional copy at the time of your exam.

4.  Can you bill our company / TPA for an exam?   

Most exams are paid at the time of service.  Billing is only available for companies enrolled in the Paramex DOT Consortium.

5.  Will I need to take a drug test.  

DOT physicals do not include a drug test. However, a urine sample is required for basic health screening (not drug testing).  In some cases, a company may also be requesting a pre-employment drug test or completing a random test at the same time as your scheduled appointment.
