Your Appointment cannot be performed earlier than was scheduled, and late appointments may be canceled to avoid a lapse into another clients appointment time.
1. Arrive on time.
2. Bring your ID
Please bring a valid driver's license. We cannot accept an expired drivers license or a photo copy of your license. Your license does not need to list you as a class B or A.
3. Payment Due
Our doctor does not bill all DMV Exams. Only DOT companies who are enrolled in Paramex' DOT Consortium will be billed for exams.
Independent Owner Operators who are enrolled with Paramex must pay for their exam at the time of service but will receive a discount.

PARAMEX WILL NOT BILL YOUR DRUG TESTING CONSORTIUM FOR A DMV.
4. Health History
You will be asked basic health history information. Please be prepared to indicate dates of onset, diagnosis, treating physician's name and address, and current limitations related to a medical condition or operation. Bring a list of current medications (including over-the-counter medications) used regularly or recently.
5. Urine Specimen
DMV exams do not require a drug test. Unless you are aware of a drug test that is needed at the time of your exam, you will not be required to take one. The doctor will need a urine sample for a basic urinalysis. Please drink a water before arriving to ensure you can provide a specimen in a timely manner.
6. Glasses
You will be taking a vision test during your exam. It is a simple check-up, but if you wear glasses while driving, you need to bring the glasses with you for this portion of the exam.
7. Diabetes
If you are diabetic, consider bringing your most recent A1c results. In addition, if you use insulin, you will need to get mcs form 5870 filled out before coming to your appointment. Our office cannot pass you if you do not bring this completed form.
Be Prepared for your Physical

1. What do I do after the exam?
The Doctor will provide you with the original copy of the exam and the signed certificate. It is your responsibility to take these documents to the DMV.
2. Can you send results to my company or another party?
Your examination, the results, and the medical report (called a long form) is all confidential material and is protected under HIPPA law. Our Doctor's office will not forward your material or exam pages even with your verbal or written permission. You paid for the exam, they are your results. You may forward as many copies to whom you would like.
3. Can I get additional copies of my DMV Certificate?
Our office will supply you with the original forms and one additional copy of your certificate.
4. Can you bill our company / TPA for an exam?
PARAMEX WILL NOT BILL unless you are a member of our consortium.
5. Will I need to take a drug test.
