As a result of your recent written health evaluation, and in the best interest of your health and personal safety; our doctor has requested you schedule a follow-up appointment at our office.
Please select from the available appointments below. You will need to provide a valid e-mail address to schedule your appointment. Once your appointment is confirmed, you will receive an instructional e-mail and follow-up reminders for your appointment date.
Your Appointment cannot be performed earlier than was scheduled, and late appointments may be canceled to avoid a lapse into another clients appointment time.

Please bring a valid driver's license. We cannot accept an expired driver's license or a photo copy of your license. Your license does not need to list you as a class B or A.
3. Payment Due
99.99% of all Respirator Evaluations are employment based. Paramex will be billing your company for the expense of today's exam.
4. Health History
You will be asked basic health history information. Please be prepared to indicate dates of onset, diagnosis, treating physician's name and address, and current limitations related to a medical condition or operation. Bring a list of current medications (including over-the-counter medications) used regularly or recently.

1. Why do I need to see a Doctor? I feel fine.
The purpose of your visit is to discuss some of the contents of your recent written medical evaluation. In some cases, the doctor has an additional question, needs to examine for himself, or fill in some missing information that may have been missed or marked incorrectly.
2. What do I need to do after the appointment?
Your exam results will be sent back to your employer. You are entitled to know the results and/or decision of your doctor. In some cases, the doctor may request that you see your personal physician or obtain treatment from their office.
3. I only speak spanish.
No worries, our doctor speaks spanish. You are welcome to bring someone with you to help you, if it will help you.
