As a result of your recent written health evaluation, and in the best interest of your health and personal safety; our doctor has requested you schedule a follow-up appointment at our office.

Please select from the available appointments below.  You will need to provide a valid e-mail address to schedule your appointment.  Once your appointment is confirmed, you will receive an instructional e-mail and follow-up reminders for your appointment date.

Your Appointment cannot be performed earlier than was scheduled, and late appointments may be canceled to avoid a lapse into another clients appointment time.

1.  Arrive on time.  

2.  Bring your ID  

Please bring a valid driver's license.  We cannot accept an expired driver's license or a photo copy of your license.  Your license does not need to list you as a class B or A.

3.  Payment Due  

99.99% of all Respirator Evaluations are employment based.  Paramex will be billing your company for the expense of today's exam.  

 

4.  Health History  

You will be asked basic health history information.  Please be prepared to indicate dates of onset, diagnosis, treating physician's name and address, and current limitations related to a medical condition or operation.  Bring a list of current medications (including over-the-counter medications) used regularly or recently.

1.  Why do I need to see a Doctor? I feel fine.    

The purpose of your visit is to discuss some of the contents of your recent written medical evaluation.  In some cases, the doctor has an additional question, needs to examine for himself, or fill in some missing information that may have been missed or marked incorrectly.   

2.  What do I need to do after the appointment?

Your exam results will be sent back to your employer.  You are entitled to know the results and/or decision of your doctor.  In some cases, the doctor may request that you see your personal physician or obtain treatment from their office.   

3.  I only speak spanish.   

No worries, our doctor speaks spanish.  You are welcome to bring someone with you to help you, if it will help you.

2020 Terms and Conditions

Contracted Physicians working with Paramex Screening Services offers respirator evaluations and follow-up/verification doctor appointments to individuals and companies who fall under health and safety regulations for respirator use.  Information transferred and stored is done by the direction and oversight of the Physician and supporting staff.  Services provided by the physician to the clients establish a doctor/patient confidentiality.  Paramex Screening Services will not use examination results for purposes outside of the doctor / patient agreement.  Additionally, Paramex Screening Services reserves the right to refuse delivery of results to a requesting party or deny services to a client on behalf of the Physician.   

 

HIPPA guidelines are followed by our doctor’s office.  A copy of HIPPA guidelines and compliance is available upon request.  An agreement to the terms and conditions releases the Physican, Paramex Screening Services, its staff, affiliates, and collectors from all liability related to standard collection and examination procedures, the delivery of examination results.

 

The Patient agrees:  I do hereby release the Physican, their representatives, Paramex Screening Services, its employees, agents, representatives, and collectors from any and all liabilities arising from the release or use of the information derived from or contained in my examination results.  I understand that no-one can access my examination results without my express written permission and that a valid copy of my state or federal ID is required if picking up additional copies of my exam pages.  I further understand that all materials written on the respirator evaluation questionnaire will not be transmitted to my employer.

 

With the intention of maintaining neutrality and unbiased relations in your examination process, The Physician and Paramex Screening Services, will not provide employment advice to employers based on results of an examination.  All advice, opinion, explanation, and medical direction given by the physician is held in confidence. Any decision to use examination results or materials as an employment decision is done at the choice of the donor and his potential or current employer.  Only the required information will be recorded and reported on the respirator evaluation recommendation form.  

 

Clients / Patients are required to maintain their appointments.  Appointments are auto confirmed.  All data in the scheduling fields are requested to be filled.  An e-mail address is required for scheduling your appointment.  A confirmation will be sent to your e-mail address to show verification that your appointment has been scheduled with our office. This email confirmation will provide a link for you to cancel your appointment in the event that you are unable to keep the date and time that you requested.  All no-show clients and appointments canceled same day will be charged an additional $30.00 if a second appointment is made.  Calling our office more than 24 hours in advance (530) 895-3203 will be acceptable only if you leave contact with a staff member.  Voicemail messages will not be accepted as a means of canceling your appointment.  Companies who are billed for respirator exams will be billed for no-show and late cancellations of exams.  Failure or refusal to pay the additional fees may result in the cancellation of services with Paramex' Physicians.
